A standard GP consultation is designed to assess and manage a wide range of everyday health concerns. It commonly includes care for acute illnesses such as colds and flu, gastroenteritis and urinary tract infections, as well as follow‑up appointments to review test results or ongoing symptoms. GPs also use these consultations to provide medication reviews and repeat prescriptions, arrange referrals to specialists, and request imaging or pathology when needed. Importantly, standard consultations are where new or early signs of chronic conditions such as diabetes, asthma or high blood pressure are first identified and a management plan is discussed.
Chronic Condition Management focuses on supporting patients living with long‑term conditions such as diabetes, cardiovascular disease, chronic obstructive pulmonary disease (COPD) and asthma, arthritis, and mental health comorbidities. Through structured care plans, regular reviews and coordinated support with allied health and specialist services, GPs help patients monitor their condition, adjust treatments and address both physical and emotional health needs. The aim is to improve quality of life, reduce complications, and provide continuity of care, so patients feel confident, informed and supported in managing their health over the long term.
Mental health identification and treatment focus on early recognition, compassionate care and ongoing support. GPs commonly identify concerns such as anxiety, depression, stress and adjustment difficulties during routine consultations and provide a safe, confidential space to talk about mental wellbeing. Care may include the development of a GP Mental Health Treatment Plan, brief counselling and practical strategies, as well as referrals to psychologists, psychiatrists or other non‑GP specialist services when additional support is needed. This coordinated approach helps patients access appropriate care, build coping strategies and improve their overall mental health and quality of life.
